 IF YOU ARE NOT A REGISTERED MEMBER OF ST. JUDE 
 A LETTER FROM YOUR PARISH PASTOR PERMITTING US TO BAPTIZE YOUR CHILD IS REQUIRED   

Process for Baptism: (in this order)               

1. Submit all paperwork and documents (see below) 

2. Parents will be called for an interview by clergy 

3. Baptism Preparation for parents & godparents 

Inform us if any of you previously attended a prep class for another child within the last 4 years

Please submit the following:

1. Baptism Application

2. Photocopy of child’s state birth certificate (if you did not receive one yet, you may submit the Hospital Certificate, but please add mothers’ MAIDEN name to document)

3. Godparent(s)/Sponsor Form(s) 1 godfather and 1 godmother only. (1 godparent must be a practicing Catholic in good standing).

4. Baptism date  WILL NOT be scheduled/confirmed until all required documents are submitted and reviewed for completion.
    (we advise that you not make any party/celebration plans before the baptism date is confirmed)
